• 3-Month Minimum Commitment: Early termination before 3 months will incur a $100 cancellation fee.
   
• Auto-Renewal: All memberships renew monthly unless written cancellation is submitted at least 7 days before the next billing date.
   
• No Rollovers: Unused services do not carry over to the next month.
   
• Non-Transferable: Membership benefits are non-transferable unless specifically stated (guest IM in Max tier only).
   
• Medical Clearance Required: All services must be approved by a licensed medical provider. Onyx reserves the right to withhold treatment if clinically inappropriate.
   
• No Refunds: Once billed, membership fees are non-refundable.
   
• Service Limits: Memberships apply exclusively to IV drips and IM injections as outlined above. Aesthetic, hormone, weight loss, and skincare services are not included.
   
• Price Adjustments: Onyx Aesthetics reserves the right to change pricing with 30 days’ written notice.
   
• Late Cancel/No-Show Policy: Cancellations within 24 hours or missed appointments may result in forfeiture of that month’s service.
